Job Type: Collaborating Per Diem Physician, VSP

Work Environment: Hybrid (Remote & Field-based) *As needed, will conduct quarterly controlled substance visits to patient’s home within the assigned region.  

Assigned Territory: Georgia – College Park, Atlanta, Forest Park, Covington, Conyers, Stonecrest, Monroe, Mountain Park, Lilburn, Tucker, Panthersville, Douglasville, and Mableton

This position is considered Per Diem, used on an as needed basis. Scheduling is flexible and can be anytime during normal hours of operation, nights, and/or weekends.

This position will provide chart review for 3-5 hours per month. Additionally, this role will require providing collaborative physician duties for up to 4 mid-levels.

The Collaborating Per Diem Physician serves as the point of contact for a team of up to 4 advanced practice providers who provide home-based primary care to our senior population. In addition to supporting our advanced practice providers, you provide conduct occasional chart review.

The Collaborating Per Diem Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgement and decision making on complex issues regarding job duties and related tasks and works under minimal supervision. Uses independent judgement requiring analysis of variable factors and determining the best course of action.

  • Provide collaborating agreements for up to 4 nurse practitioners in the state of Georgia including but not limited to consulting with assigned nurse practitioners on management of complex patients.

  • Review all acute utilization events, with goal of reducing unnecessary utilization.

  • Use of laptops for EMR charting.

  • As needed/ongoing attention to prescription refills, home health referrals/co-signatures, lab follow-ups and coordinated care management.

  • As needed, quarterly controlled substance visits to patient’s homes.

Required Qualifications

  • Minimum of 2 years relevant experience conducting chart reviews and collaborating agreements consulting with a team of 4+ advanced practice providers.

  • Graduate of accredited MD or DO program of accredited university.

  • Must have an active/unrestricted Georgia state license, DEA license, and board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.

  • Senior patient care experience.

  • Active BLS through the AHA (American Heart Association) or ARC (American Red Cross).

  • Strong technical experience.

  • This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences.

  • Dedicated to providing excellent healthcare and a top-quality experience.

  • Strong belief in the revolutionary nature of house calls and passionate about improving care delivery.
